New WHO HIV Treatment Guidelines

The World Health Organization has released updated recommendations on HIV clinical management, marking the most significant advance since the consolidated guidelines of 2021. These updates reflect growing evidence that HIV care must be simpler, safer, and more responsive to the realities of people living with HIV.

Central to the new guidance is the continued preference for dolutegravir-based antiretroviral therapy, recognised for its effectiveness, tolerability, and ability to support long-term viral suppression. WHO also introduces more flexible, patient-centred options, including improved choices for second-line therapy, support for long-acting injectable treatments, and simplified regimens for people who struggle with daily pills.

Tuberculosis prevention remains a major focus. WHO now recommends a shorter, three-month preventive treatment regimen (3HP) for people living with HIV — a move expected to improve completion rates and reduce TB-related deaths.

For countries like Nigeria, these recommendations signal a shift toward evidence-based, cost-effective, and people-centred HIV care that health systems can better sustain and scale.
